چکیده

TOPIC: Imaging TYPE: Original Investigations PURPOSE: B-lines on lung ultrasound are an early finding in patients with decompensated heart failure. Hence, monitoring failure their home environment could help to prevent hospital admissions. The aim of this project is investigate whether tele-guided patient-performed the feasible. METHODS: Stable ambulatory NYHA class 1-3 were included study. Patients received a hand-held probe connected tablet (patients 1 3) or smartphone. Initial instructions given during either clinic visit remote visit. each performed between 10 and 12 daily sessions, interrupted by weekends occasional days unavailability. During session, guided via verbal visual remotely physician. Over side chest, 3 locations within 3rd intercostal space examined, moving from mid-clavicular line anterior mid axillary line. also reported weight level dyspnea, graded 5-point scale. Ultrasound clips reviewed two independent reviewers, as interpretable if pleural plus at least one A-line any visible. Gain depth appropriate inappropriate. RESULTS: Eight patients, 2 them female, total 89 sessions. There no major technical problems, 88% (reviewer 1) 84% 2) all interpretable. A 92% 90% exams had 4 out 6 clips. Inter-reviewer agreement was substantial 93% observations Cohen’s Kappa 0.7. More than 97% gain settings. No patient abnormal exam, none admitted for exacerbation. Most (6 8) stable dyspnea scores throughout. score more 3% above dry weight. CONCLUSIONS: Tele-guided High quality obtained, evidenced being diagnostically usable decompensations observed, false positive recorded. To determine technology can be used detect decompensations, unstable need longer study period. CLINICAL IMPLICATIONS: This have far-reaching diagnostic implications, not just patients. Respiratory failure patient with lung cancer diagnosed by transesophageal bronchoscopic ultrasound‐guided aspirates

A 71-year-old man, who had received long-term oxygen therapy for respiratory failure caused by chronic obstructive pulmonary disease, had an enlarged mediastinal lymph node for one year. As his lung function was poor, we tried performing endobronchial ultrasound-guided transbronchial needle aspiration under non-invasive positive pressure ventilation for diagnosis but could not obtain sufficient...

Feasibility of surgeon-performed ultrasound-guided core needle biopsy in the thyroid and lymph nodes.

BACKGROUND The purpose of this study was to evaluate the feasibility of ultrasound-guided core needle biopsy (CNB) performed by a surgeon for mass lesions in the thyroid and lymph nodes. METHODS A single surgeon performed 30 office-based ultrasound-guided CNB procedures for mass lesions in the thyroid and lymph nodes that were previously biopsied by ultrasound-guided fine-needle aspiration cy...

Diagnosing acute heart failure in patients with undifferentiated dyspnea: a lung and cardiac ultrasound (LuCUS) protocol.

OBJECTIVES The primary goal of this study was to determine accuracy for diagnosing acutely decompensated heart failure (ADHF) in the undifferentiated dyspneic emergency department (ED) patient using a lung and cardiac ultrasound (LuCUS) protocol. Secondary objectives were to determine if US findings acutely change management and if findings are more accurate than clinical gestalt. METHODS Thi...
